  The "Average" row of this table contains numbers, which are inaccurate. Here is the revised table:
   

• Page #269, Example 8.2
  All numbers in Table 8.14 are incorrect. They are based on 4 raters, the 3 raters and the "Consensus" rater of Table 8.13. This table is supposed to be based solely on the 3 raters. The "Consensus" rater is excluded from the calculations. The corrected table, along with the revised comments are the following:
